Job Description:
A company is looking for a Care Coach II to support family
caregivers and their care recipients.
Key Responsibilities
Engage, motivate, and educate caregivers through phone and digital
technology
Manage a caseload and assess caregiver and care recipient needs to
create care plan goals
Document coaching activities and collaborate with stakeholders to
improve care plans
Required Qualifications
4-6 years of experience in health coaching or
healthcare/wellness
Comprehensive knowledge of industry concepts and principles
Solid understanding and experience in care planning and case
management
Spanish fluency is highly desired
Able to work 10-7 EST
